The Admission & Referral Process
Admissions:
The Admission Process begins with the initial intake, conducted through a call or visit with the client or their family. During this stage, we inquire about relevant details and gather the necessary information to progress to the subsequent steps. All the information gathered is treated with the utmost confidentiality and is kept in-house. We adhere to a strict policy of not sharing private data with anyone unless we have obtained written consent from the client or a family member.
We will arrange for an In-Home Assessment to evaluate the client and survey the home for potential hazards or safety concerns. In compliance with Pennsylvania HHCS regulations, Intercede is required to complete this assessment before the commencement of care to ensure the best possible service. If the client is currently in a hospital, rehabilitation center, or nursing facility, we will conduct the assessment at the facility and subsequently visit the home for a Safety Inspection.
For a smooth process, please bring copies of your Advance Directive, OOH-DNR, Medical POA, a list of your current medications, and identification, along with your health insurance card (not required for private pay).
After signing the Consent for Treatment, our scheduler will contact you to establish a caregiver schedule tailored to your specific needs. Our caregivers will be scheduled to arrive at your home on the designated days to provide the necessary care.
Referrals:
We currently receive direct referrals from Discharge Planners, Social Workers, Nurses, Doctor’s Offices, Hospitals, and Nursing Homes.
Referrals from Support Groups, Churches, Friends, and Family members are greatly appreciated but, we accept referrals from everyone.
Referrals may be called, emailed, messaged, or faxed.
The more information we receive, the more efficiently we can provide care, and below are the basic demographics required such as name, address, age, and major diagnoses (i.e., hip fracture, dementia).
